Sažetak:Introduction: Adapted physical activity (APA) has a recognized impact on reducing complications in chronically ill or frail subjects. Since 2021, the Vivoptim scheme has been offering patients with long-term conditions (LTCs) a one-year care package including digital services, interviews with a multidisciplinary team of healthcare professionals, and a reimbursed APA program. Our aim is to demonstrate the positive impact of this scheme on the health behaviors and increased physical activity of LTC patients. Methods: 130 subjects (women: 84%, average age : 62 years) had completed a fitness assessment (BCP) upon inclusion and were monitored for one year. Physical activity, stress, sleep quality, and weight were assessed by questionnaire. Frailty was estimated using BCP tests. The results observed were compared using McNemar and Student’s t tests. Results: After one year, we observed among the mostly frail patients (57%) increases in the proportions with moderate physical activity over 2h30/week (85% vs. 60%, p<0.001), good sleep quality (+13%, p<0.05), and low stress levels (+14%, p<0.001). 55% of subjects were motivated to continue pursuing APA after this support. Conclusion: The Vivoptim phygital tool has proven its effectiveness in increasing physical activity and improving health behaviors among LTC patients. The prescription of APA, systematically including standardized BCPs at the beginning and end of APA practice, is essential.
