Summary:While in some countries, such as Belgium, people with mental disorders can now obtain a decision to undergo euthanasia if their unbearable suffering is certified as an incu-rable disease, the recovery paradigm emphasizes the possibility of a favorable outcome, regardless of the severity of the disorder. This raises the question of how clinicians experience unbearable suffering, to which their knowledge seems powerless. We first put the notion of psychological suffering back in perspective to the subjective universe of the person expe-riencing it, which leads us to distinguish between actual psychological pain and fundamental suffering as an alteration in the potential of the subjective universe. After introducing the Untenable as the extreme point of fundamental suffering, we show how the recovery para-digm can offer a framework for overcoming such an ordeal. We then clarify the notion of the Unbearable, as the Untenable induced by the suffering of Another, before considering how a carer can be confronted with it. We argue that there is never a point of no return for the Untenable in the psychic universe, nor terminal knowledge in mental pathology, the psychic being by essence potential for openness in each encounter.
