Cette étude vise à élaborer un test qui : 1) peut être administré au chevet ; 2) évalue brièvement l’ensemble des fonctions cognitives pertinentes ; et 3) est sensible aux séquelles d’un TCC de différent degré de sévérité.Participants et méthodeL’EXACT comprend 22 sous-tests évaluant brièvement cinq domaines cognitifs. Il a été administré à 169 participants témoins et 307 patients hospitalisés pour un TCC léger à sévère. Le Mini-Mental State Examination (MMSE) a aussi été effectué par un sous-groupe de participants et l’échelle d’incapacité fonctionnelle (DRS) a été administrée un an après l’accident.RésultatsLa performance à l’EXACT est corrélée avec le score au MMSE et à la DRS. De plus, les résultats varient significativement selon l’âge et la sévérité du TCC. La spécificité de l’EXACT est comparable à celle du MMSE, mais sa sensibilité est nettement supérieure. Les items évaluant l’attention et les fonctions exécutives semblent les plus sensibles.ConclusionL’EXACT constitue un outil valide et fidèle pour évaluer brièvement le fonctionnement cognitif des patients jeunes et âgés durant la phase aiguë d’un TCC.ObjectiveThe role of the clinical neuropsychologist in tertiary trauma center hospitals is to quickly assess the global cognitive functioning of patients who have sustained a traumatic brain injury (TBI) in order to determine their orientation at the time of discharge. However, the few global cognitive instruments available for this clinical purpose present certain limits. The aim of this study was to design a new test that (1) can be administered at the patient's bedside; (2) briefly assesses all relevant cognitive functions; and (3) is sensitive to the sequelae of different TBI severity levels.Participants and methodsEXACT is composed of 22 subtests (maximum score of 100 points) that briefly assess five cognitive domains: (1) instrumental functions, (2) language, (3) attention and working memory, (4) episodic memory and (5) executive functions and behavioral regulation. This test can be performed in 15 to 60 minutes. It was administered to 169 normal control participants (aged 18 to 71 years) and 307 patients (aged 17 to 76 years) hospitalized for a mild to severe TBI. A subgroup of participants ( n = 368) also underwent the Mini-Mental State Examination (MMSE). The Disability Rating Scale (DRS) was used one year after the accident.ResultsPoorer performance on EXACT was associated with lower MMSE scores and a higher level of disability on the DRS. The EXACT results varied significantly according to age and the severity of the TBI. The specificity of this new test was comparable to that of the MMSE, but its sensitivity when it came to detecting cognitive impairment after a TBI was much higher. Items assessing attention and working memory (domain 3) and executive functions and behavioral regulation (domain 5) were the most sensitive to a TBI.ConclusionEXACT is a valid and reliable brief clinical test that can be used to assess global cognitive functioning during the acute phase of a TBI. of a TBI.Cairn free access
