520 |a Recently, robotics has found a new kind of research field in surgery in which it is used as an assistant of the surgeon by giving more information on the state of the device to the surgeon, by augmenting wave (precision, tasks ) or by performing tasks that allow the surgeon to keep his mind on his task. To the requirement of ophthalmologists, we propose a device to perform penetrating keratoplasty (corneal grafting) following the new techniques suggested in order to decrease the postoperative astigmatism that occurs and by this way, to increase visual recovering. The surgery consists in taking donor s and receiver s corneas by manual trephination. The graft is then put in place and sutured to the patient s cornea by a continuing suture. These two steps and the tools used create a postoperative astigmatism. In order to limit astigmatism due to the trephination, we propose to cut corneas with a low pressure surgical water jet. Feasibility has been proved and parameters established. In order to decrease astigmatism due to the suture, we propose to replace it by a conical shape of cut including tabs followed by the suture of a large contact lens to the patient s sclera in order to be sure of the good tightness to the grafting. A surgical robot has been designed to assist the surgeon during the surgery as it was just defined. An analysis of the risks that might happen and the specifications has been determined. We chose a Scara type structure which is put on a truck and which holds a specifically designed tip. An extensive study must be carried on the pulsed water jet cutting in order to significantly reduce hydration of the cornea that occurs. Also, some trials will have to be carried to validate the propositions made after the prototype is made.
